The Chronic Care Summit · Cape Coast 2026

You feel fine. Know where you stand.

Join us at Amissah Arthur Hall, University of Cape Coast for free screening, practical health conversations and an introduction to continuing support.

Why we screen

Most people find out too late.

27%

of adults in Ghana are living with high blood pressure — and only about a third of them know it.

Bosu & Bosu, PLOS ONE, 2021 — pooled analysis of 85 Ghanaian studies

73%

of adults living with diabetes in Africa have never been diagnosed — the highest proportion of any region in the world.

IDF Diabetes Atlas, 2024 (adults 20–79)

Your day at the summit

Come curious. Leave supported.

You do not need to feel ill to check where you stand. Drop in for screening, ask questions and meet the people who can support your health for the year ahead.

The Fellowship

See how year-round, coordinated support works after the summit ends — including our specialist partnerships in Ghana and India, and how SafaraCare supports treatment abroad where that is clinically appropriate.

For health facilities

Partner with us in Cape Coast.

Screening does not end at the screening table. Everyone who learns their numbers at the Summit leaves with a next step — a confirmatory test, a clinical review, medication to manage, a reason to start moving. The facilities those residents find will be the ones that were there.

Every Partner Facility gets its own interface on the Freedom Protocol platform: onboard your own patients, and see laboratory orders placed by members' doctors in your city. Staff training happens at the Summit itself, and partners are recognised and introduced to attendees on the day.

Who we are looking for

  • Laboratories & diagnostic centres
  • Private hospitals & clinics
  • Pharmacies
  • Gyms & fitness centres
  • Physiotherapy & massage practices
  • Nutrition & dietetics practices

What we ask of partners

  1. Confirm as early as you can, so a training place can be arranged before the Summit.
  2. Nominate one or two staff to attend the Summit and the partner training session.
  3. Bring a copy of your facility's current operating licence — from the Health Facilities Regulatory Agency, the Pharmacy Council or your relevant regulator — so your partner account can be set up on the day.
